Procedures are written down, but how they are applied varies from one person to another, and nothing controls that interpretation.
Knowledge stays oral
The exceptions are real, they apply often, and only two people know them: they appear in no document. When a knowledge holder leaves, what remains are documents that nobody can arbitrate any more.
